17 Fermanagh and Omagh District Council Community Planning Strategic Partnership Board Integrated Wellbeing Network Age Friendly Alliance Fermanagh Omagh Interagency Forum South West Age Partnership It is further recognised that there are a range of initiatives being taken forward by partners operating within the health and wellbeing sector, including Fermanagh and Omagh Community Planning through its Integrated Wellbeing Network and the Department of Health Integrated Care system to note just some. Key to governance and implementation is embedding clear communication between these approaches and promoting collaboration where appropriate. Monitoring and evaluation of progress is also essential in assessing the success of our efforts in establishing an Age Friendly District in Fermanagh and Omagh. In addition to applying the population indicators used in Community Planning, proposed delivery partners will be responsible for monitoring and evaluating the progress of each action set out in the plan overleaf. This progress will be reported back to the Age Friendly Alliance as a regular agenda item to ensure ongoing review and the opportunity to revisit any actions as required.
