24 Age Friendly Strategy and Action Plan AIM ACTION - How will it be Addressed? Accessing hospital appointments Explore potential for an option for later hospital appointments if preferred for those involving long journeys Living Well (Frailty Prevention) Delivery of physical activity programmes for older people in local community venues Tackling poverty Support wider work to address poverty among older people to alleviate food and fuel poverty and encourage older people to take up pension credit Promote positive outcomes for older people PSNI officers refer vulnerable older people into the PSNI vulnerability support team for assessment, offering additional support and signposting to the most appropriate agency Delivery of the Co-Creating Hope initiative, designed to provide more integration in meeting the needs of elderly and frail residents in the FODC area. This will include the PEACEPLUS GRANT FUNDED for Three Years - Early Frailty Intervention Project (EFIT) made up of community based Professional staff established in cross border localities, to identify people living with (or at risk of developing) frailty earlier. The project will provide a triage for frailty risk and consultation, after which a programme of care for screening/advice/education/ community-based support. The project will target 2,200 beneficiaries over a three-year period across Western Trust areas, Derry Omagh and Fermanagh. 8 staff will be employed within WHSCT area based in Derry and Omagh. THEME: HEALTH AND WELLBEING The theme recognises that access to and availability of services is crucial in maintaining health and independence among older people and supporting sustainable communities. It is recognised that the action plan and associated activities must be measurable in order to monitor progress and impact across the life of the Strategy and Action Plan.
