14 I Age Friendly Strategy 2026 - 2030 Age Friendly Strategy 2026 - 2030 I 15 THEME 3: TACKLING ISOLATION An Age Friendly community enables people of all backgrounds to actively participate and treats everyone with respect, regardless of age and supports actions to build respect and promote opportunities for inclusion by older people. AIM ACTION - How will it be Addressed? Building inclusion by older people at group and individual level 3.1(a) Capacity building to sustain the sector, including SWAP Cluster meetings and focus groups 3.1(b) Work with the Policing and Community Safety Partnership on safety programmes tailored for older people 3.1(c) Promote volunteer recruitment and recognise volunteers 3.1(d) Promote scam awareness, tailored for older people 3.1(e) Promote services and support for people living on their own Ensuring information is accessible to all 3.2(a) Providing literature in printed form as well as digital Alignment with Community Plan: Outcome 2: Older people lead more independent, engaged and socially connected lives Outcome 3: Our communities are inclusive, safe, resilient and empowered THEME 4: HEALTH ANDWELLBEING The theme recognises that access to and availability of services are crucial in maintaining health and independence among older people and supporting sustainable communities AIM ACTION - How will it be Addressed? Increase awareness of local health care and wellbeing provision / initiatives 4.1 Provide information on the provision of local health care and wellbeing initiatives Dementia Awareness 4.2 Increase awareness of dementia support services in the district Accessing hospital appointments 4.3 Explore potential for an option for later hospital appointments if preferred for those involving long journeys Living Well 4.4 Delivery of health and wellbeing, including physical activity and falls prevention for older people Tackling poverty 4.5 Support wider work to address poverty among older people to alleviate food and fuel poverty and encourage older people to take up eligible benefits including pension credit Promote positive outcomes for older people 4.6(a) PSNI to support older people through early identification of vulnerability, safeguarding intervention, crime prevention activity, and partner-based support and sign posting 4.6(b) Early Frailty Intervention Project (EFIT) made up of community based Professional staff established in cross border localities, to identify people living with (or at risk of developing) frailty earlier Alignment with Community Plan: Outcome 1: Our people are healthy and well - physically, mentally and emotionally Outcome 2: Older people lead more independent, engaged and socially connected lives Outcome 3: Our communities are inclusive, safe, resilient and empowered It is recognised that the action plan and associated activities set out in the annual implementation plans must be measurable in order to monitor progress and impact. https://qrco.de/FODC-Age-Friendly and by contacting the Age Friendly Officer.
